Core Benefits

Medical Insurance—Full-time or otherwise qualifying employees are eligible to participate in our group medical health benefit with Priority Health on the 1st day of the month after completing 30 days of service. There are currently two plan choices available: (1) a high deductible Health Reimbursement Account (HRA) in which Hospice pays a significant portion of the deductible, and (2) a high deductible plan with a Health Savings Account (HSA), which enables employees to save pre-tax dollars into a personal health savings account that can be used for deductibles and other qualified health care expenses. A wellness discount is available on both plans. Both plans require employee participation in the cost of the premiums.

Dental/Vision—Full-time or otherwise qualifying employees may purchase dental and vision insurance for themselves and their families.

Life Insurance—Full-time or otherwise qualifying employees receive a $15,000 life insurance policy with a $15,000 AD&D coverage paid for by Hospice of Holland.

Long Term Disability—All full-time or otherwise qualifying employees are provided with Long Term Disability coverage at 60% of their pre-disability monthly earnings effective after 90 days of disability. This coverage applies until able to return to work or retirement age, whichever comes first. The coverage is paid for by Hospice of Holland.

401k Plan—Hospice of Holland employees are auto-enrolled into the 401k plan after 90 days of employment. Employees are eligible for a 4% match at the beginning of the quarter following one year of employment.

Flexible Spending Accounts (FSA)—Employees working an average of 30 hours a week may participate in our FSA program. This plan allows employees to set aside pre-tax wages for reimbursement of child care or healthcare expenses that are not covered by any health plan.

Earned Time Off (ETO)—Full-time and part-time employees accrue paid time-off for vacation, sick time, and personal days based on hours worked. Employees are able to use ETO once they have worked 90 days.

Holidays—Full and part-time staff receive holiday pay for recognized holidays after 90-days of employment. Pay is prorated, based on the number of hours you are scheduled to work on a weekly basis. Currently there are 6 designated holidays: New Year’s Day, Memorial Day, The 4th of July, Labor Day, Thanksgiving Day and Christmas Day.

Bereavement Leave—Employees receive up to 3 paid days in the event of the death of an immediate family member.

Jury Duty—In the event an employee is called to jury duty, Hospice of Holland pays their salary minus what is earned as a juror for ten days.

Supplemental Benefits

Supplemental Life/Dependent Life—Full-time or otherwise qualifying employees may purchase up to three times their annual salary in life insurance ($150,000 maximum), up to $75,000 life insurance for their spouse, and up to $10,000 for each dependant child.

Short Term Disability (STD)—Full-time or otherwise qualifying employees may purchase STD which replaces up to $500.00 of salary a week in the event of disability lasting 8 days to 13 weeks.

  • Employees can opt out of auto-enrollment during orientation and at any time within 90-days of being auto-enrolled.

  • You do not need to be enrolled in one of our medical plans, so long as your spouse is enrolled in a qualified medical plan.

All full-time or otherwise qualifying employees are provided with Long Term Disability coverage at 60% of their pre-disability monthly earnings effective after 90 days of disability.
