Medical Social Worker – PRN (Weekends)

Job Description Summary

Work as a member of the Interdisciplinary team to assist patients and families in managing the psychosocial, emotional and resource needs related to end of life issues.

Essential Job Functions

  1. Completes admission and transfer process with patients and families including explaining hospice services and insurance benefits and completing paperwork as appropriate.
  2. Assesses the psychosocial status of patients/families/caregivers related to the patient’s 
terminal illness and environment. Participates in development, implementation, evaluation and updating of Interdisciplinary Care plan.
  3. Provides counseling and education for patients and families and community members regarding psychological, emotional and social aspects of death and dying.
  4. Assists patients and families in identifying and utilizing appropriate resources to meet environmental, financial, legal or psychological needs.
  5. Participates in weekly Interdisciplinary Team Meetings; attends facility care conferences.
  6. Coordinates and facilitates Care Conferences.
  7. Documents all patient/family contact and activity in an accurate, professional and timely manner.
  8. Provides on-call support for patients and families as scheduled.
  9. Participates in Hospice and community programs as requested to promote understanding of Hospice philosophy and care and to educate the general public about psychosocial aspects of death and dying.
  10. Operates within parameters of Hospice of Holland, Inc. and state, federal and other regulatory requirements.

Job Qualifications

Education and Experience

  1. Education: Master’s degree in social work from a school accredited by the Council on Social Work Education is required.
  2. Experience: Experience in counseling, health care, Hospice and/or family systems is preferred.
  3. Other: Current Michigan Social Work licensure as a clinical social worker. Current Michigan driver’s license and available transportation.

Works under a general and somewhat removed control which manifests itself directly only on limited occasions. Such phases as planning and organizing the details of work and deciding the methods to achieve a given result are completely in the hands of the employee. The employee is expected to carry out not only all the ordinary duties of the position, but to meet unusual situations and provide proper interpretations without advice or instruction other than that afforded by the general plans, policies and procedures applicable to all work in the department or organization. Continuing Education Requirements

Hospice personnel are expected to participate in appropriate continuing education as may be requested and/or required by their immediate supervisor. In addition, Hospice personnel are expected to accept personal responsibility for other educational activities to enhance job-related skills and abilities.
