Job Description Summary

Responsible for managing all aspects of patient intake, referral, and admission scheduling process.

Essential Job Functions

  1. Manages all patient referral, intake, and ​admission scheduling operations, including maintaining
    accurate records for admitted, postponed, cancelled and pending referrals.
  2. Coordinates the communication of pertinent information with the interdisciplinary team and
    physicians during the referral and admission process.
  3. Supports the interdisciplinary team through patient data entry, documentation and record keeping.
  4. Gathers, collates, and reports referral and admission statistics.
  5. Works with Admissions Registered Nurses (ARNS) to coordinate Hospice admissions and maintain customer service with referral sources.
  6. Establishes and maintains positive working relationships with current and potential referral sources.
  7. Ensures compliance with the policies of Hospice of Holland, Inc. and with all state, federal, and other referral and admission requirements. Assists in developing, implementing, and evaluating the effectiveness of Hospice admission policies, in coordination with Admissions Manager and the Director of Business Development.
  8. Maintains comprehensive working knowledge of community resources and assists patients in accessing community resources should the patient not be admitted to Hospice care.
  9. Attends staff meetings and department functions as appropriate.

Job Qualifications

Education and Experience

  1. High school diploma required with some continued education preferred.
  2. Minimum of two years experience in a support function where frequent customer contact is a vital part of role. Work in a medical or Hospice setting preferred.
  3. Excellent communication and public relations skills.
  4. Basic computer skills in Word and Excel and the ability to type.
  5. Able to work as part of an interdisciplinary team. Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.

Working Conditions

Continuing Education Requirements

Hospice personnel are expected to participate in appropriate continuing education as may be requested and/or required by their immediate supervisor. In addition, Hospice personnel are expected to accept personal responsibility for other educational activities to enhance job-related skills and abilities. Must complete Hospice Orientation Training (HOP)sessions as determine by their supervisor.
